Memorial and Tribute Giving

There are times when a special friend, beloved family member or compassionate caregiver inspires in us a desire to acknowledge and pay tribute to that individual and relationship in a distinctive and meaningful manner.

One such meaningful expression is a charitable gift made in honor or memory of a special friend or family member that can be directed to support Hospital programs or services that were significant in the life of that individual.

As with all gifts, a personal acknowledgment is sent to the donor. Additional notification of the gift is sent to the person being honored, or the family of the loved one being remembered (without reference to gift amounts).

To make a Memorial or Tribute gift, you may:

1. Mail your check to the Martha Jefferson Hospital Foundation office. (see address below) with a personal note (or downloadable form) that includes:

•  Name of individual(s) being remembered/honored.
• Name and mailing address of person to be notified of your gift
• Any special designation for the use of the gift

2. Call the Foundation Office at 434-654-8258 with your gift and credit card information.

Memorial contributions may be made to Martha Jefferson Hospital, c/o MJH Foundation, 459 Locust Avenue, Charlottesville, VA 22902. NOTE: Families may choose to direct memorial gifts to support Hospital programs or services that were significant in the life of the individual remembered. Families are asked to notify the Foundation office of these special requests.
